ADHD and sensory overload: why environment matters

Someone is trying to write a report in an open-plan office.

They can hear two conversations, a ringing phone, someone chewing, the ventilation system, and a faint electrical hum. The overhead light flickers just enough to remain perceptually present. Their chair feels wrong. A colleague’s perfume arrives before the colleague does.

The person keeps losing their place.

From the outside, this looks like distractibility. Perhaps they are told to try harder, use a planner, switch off notifications or practise focusing.

But 'distractible' describes what another person can see. It does not tell us what the nervous system is being asked to process.

The problem may not be a failure to pay attention. It may be how much attention is required to survive the room.

Sensory processing and ADHD

Sensory differences are commonly associated with autism. They are discussed much less often in relation to ADHD, whose familiar description centres on attention, impulsivity and hyperactivity.

Yet growing research suggests that ADHD has a significant sensory dimension of its own.

A 2026 systematic review by Dunn, Hodgson and Petty examined self-reported sensory experiences among people diagnosed with ADHD. The researchers excluded autistic participants so that the findings could not simply be attributed to co-occurring autism.

Across 13 studies, ADHD participants reported sensory differences involving sound, vision, smell, taste, touch, temperature and pain. These included both heightened and reduced sensitivity.

A larger 2025 systematic review and meta-analysis similarly found that people with ADHD showed considerably more sensory sensitivity, sensory avoidance, sensory seeking and low sensory registration than comparison groups.

This does not mean every person with ADHD has the same sensory profile.

Quite the opposite.

How sensory differences can show up in ADHD

It can seem contradictory for someone to be highly sensitive to certain sensations while missing others. But sensory processing is not a single volume control governing the whole body.

A person might:

  • hear a distant electrical hum but not notice someone calling their name;
  • be distressed by clothing seams but fail to register temperature changes accurately;
  • need music or movement to concentrate while becoming overwhelmed by unpredictable conversation;
  • seek strong flavours but avoid perfumes;
  • tolerate a loud concert while struggling with quieter sounds they cannot control.

Researchers commonly organise these patterns into four broad areas:

  1. Sensory sensitivity: noticing input quickly or experiencing it intensely.
  2. Low registration: not noticing some input until it becomes strong, prolonged or impossible to ignore.
  3. Sensory seeking: actively using movement, sound, touch, flavour or other input to create a workable level of activation.
  4. Sensory avoidance: reducing or controlling input to prevent discomfort, confusion or overload.

The same person can experience all four. Their needs may also change depending on the sensory channel, activity, environment, and available capacity that day.

'More stimulation' and 'fewer distractions' are therefore both too crude to be universal ADHD advice.

Why ADHD distractibility may be sensory

In ordinary language, distraction implies that something irrelevant has captured attention.

But the nervous system partly decides relevance.

If a sound remains perceptually prominent, the person cannot make it irrelevant through moral determination. If several sources of speech cannot be separated easily, "Ignore the background conversation" may be less an instruction than a description of the neurological ability the person is currently missing.

Sometimes the person is not attending to too little. They are attending to too much.

At other times, low registration creates the opposite difficulty. The person may need stronger, clearer or more immediate input before something becomes available to attention. Movement, music, chewing, fidgeting or predictable background sound may help some people maintain a workable state.

This can explain an apparently puzzling pattern: “I cannot concentrate because there is too much noise, but I also cannot concentrate in silence.”

The person may need input that is sufficiently stimulating but also predictable. A steady sound may organise attention. Irregular sounds may pull it apart repeatedly.

The issue is not simply the quantity of stimulation. It is the type, timing, predictability, controllability and meaning.

What we know about ADHD and sensory processing

It is tempting to explain all of this by saying that ADHD brains have 'broken filters'. That metaphor may describe someone’s experience, but it risks pretending that the underlying mechanism is more settled than it is.

The current evidence establishes group-level differences in reported sensory processing. It does not establish one neurological mechanism shared by every ADHD person.

Nor does every sensory difficulty necessarily arise from ADHD. Migraine, trauma, anxiety, hearing or visual differences, physical illness, medication, sleep deprivation and other neurodevelopmental or medical factors may also affect sensory experience.

The clinically responsible position is:

The experience is real. Its mechanism and meaning remain open to enquiry.

That is curiosity, not disbelief.

How the environment can affect ADHD

The 2026 review found that sensory differences were associated with barriers to participation, particularly in work and higher education.

This matters because it changes where we locate the problem.

Consider two working environments:

In the first, someone works under fluorescent lighting, surrounded by telephone calls, movement, unplanned interruptions and competing conversations.

They spend much of their cognitive capacity monitoring, filtering or recovering from sensory input.

In the second, they control the lighting, use headphones, move when needed and arrange focused work during quieter periods. The same person may appear far more attentive, organised and emotionally regulated.

Their ADHD has not disappeared.

The environmental mismatch has been reduced.

Disability can therefore be situational without being imaginary. A person may function well in one setting and become significantly impaired in another because the sensory costs are different.

This also helps explain why generic accommodations can fail. A quiet examination room may support someone who is easily overwhelmed by sound. For another person, complete silence removes the predictable stimulation they use to organise attention.

An accommodation is not effective because it appears calming to an outside observer. It is effective if it improves this person’s participation without creating a greater cost elsewhere.

Sensory overload and emotional wellbeing

Unrecognised sensory load rarely remains purely sensory.

As capacity reduces, a person may become:

  • less able to follow speech
  • slower to answer
  • more irritable or abrupt
  • restless
  • unable to make decisions
  • desperate to leave
  • temporarily unable to tolerate touch or conversation
  • exhausted long after leaving the environment.

Other people may interpret this as rudeness, lack of interest, laziness, poor motivation or emotional withdrawal.

The ADHD person may make the same moral judgement about themselves:

  • “Why can everyone else cope with this?”
  • “Why am I so difficult?”
  • “Why can’t I just concentrate?”

Repeated often enough, an inaccurate sensory formulation becomes a character indictment.

Understanding the mechanism does not erase the impact of snapping at someone, missing information or abandoning a task. It does, however, make useful accountability possible. 

"You need to try harder" offers nowhere to go.

“I lose access to language after forty minutes in that environment” gives us something we can investigate and design around.

How counselling can support people with ADHD

Counselling is not sensory assessment or sensory-integration treatment. But therapy can help people recognise the relationship between environment, nervous-system state, meaning and behaviour.

Instead of beginning only with, “What thought distracted you?”

We might also ask:

  • What sounds, lights, smells, textures or movements were present?
  • Which input remained in the foreground?
  • What did you fail to notice until it became intense?
  • Were you under-stimulated, overloaded or both?
  • Did you have any control over the sensory environment?
  • What happened to speech, decision-making and emotional regulation as the load accumulated?
  • What did recovery cost afterwards?
  • What changed on the occasions when the same task was easier?

This is not about attributing every difficulty to sensory processing. It is about including sensory information in the formulation rather than treating it as an irrelevant footnote.

Therapy may then help someone:

  • replace shame with a more accurate explanation;
  • identify early signs of overload or under-stimulation;
  • distinguish sensory protection from anxiety-based avoidance;
  • communicate needs without presenting them as personal failings;
  • negotiate adjustments at work, home or in relationships;
  • plan decompression before expecting further social or cognitive performance;
  • experiment with support while retaining the right to reject what does not help.

The experiment matters. Sensory needs are highly individual, and a theoretically sensible adjustment can still be wrong for the person using it.

Try a simple sensory audit

When concentration repeatedly collapses, choose one recurring situation and examine it without immediately trying to fix yourself.

Ask:

  • What was my nervous system processing? Include background input, not only the official task.
  • Was I seeking, missing, enduring or escaping input? More than one may be true.
  • Which variable could be changed? Lighting, sound, seating, clothing, movement, timing, predictability, duration or control.
  • What happened when that variable changed? Look at concentration, accuracy, irritability and recovery – not only whether the environment felt pleasant.
  • Is the support portable and sustainable? A strategy that consumes more executive capacity than it saves is not much of an accommodation.

The aim is not to engineer a perfectly controlled world. The aim is to stop demanding that the nervous system treat every environment as equally affordable.

What are the limitations of the research?

Research into ADHD sensory processing remains less developed than it should be. The 2026 review included only 13 studies, many with small samples and inconsistent reporting. Older adults were under-represented, longitudinal research was limited, and relatively few studies directly examined participation in ordinary life.

We should not turn emerging evidence into a rigid new stereotype of what ADHD 'really' looks like. But the existing findings are strong enough to change the questions we ask.

When an ADHD person cannot concentrate, the answer is not always another productivity technique. Sometimes the useful intervention is to look around the room.

Before asking, “Why can’t you pay attention?”, ask:

“What is attention already being spent on here?”
