Confidentiality Statement
The services offered by Clare Dangerfield actively aim to respect your right to confidentiality. All personal information is held in the strictest confidence and in line with ICO (Information Commissioners Office.) This means that the content of any individual / group counselling sessions will not be shared outside the session other than for supervision purposes. The only exception to this is if I feel that you or someone is at risk or potentially in danger. If this happens I may be obliged to approach the appropriate bodies. Should this be the case, I will consult you first. Other exceptions to this non sharing code are:
• If I think you are at risk of giving or receiving serious harm from self or others
• If I am required to do so by a UK court of law
• If I believe you will cause serious physical harm to yourself or another person then I will not be able to retain confidentiality and will be forced to inform my supervisor and your doctor / emergency services
• Confidentiality would be broken in the event of a breach of national security

The following explains what data I keep and why I need to do so:
Name and age- this is basic information that helps me to get to know you
Address, email address, phone number- I use this as a way of contacting you regarding sessions. I will mainly use the method you first contacted me on but if I can’t reach you I will try a different method.
GP Details- If I was worried that you were at risk then I may need to contact your GP or emergency services, I will inform you before doing so.
Session notes – I keep very brief anonymous notes of our sessions.
Will I share your data and if I do who will I share it with and for what purpose?
It is very unlikely that I will share your data. I will not sell it on or use it for unethical reasons. I may have to share data if my notes are subpoenaed by court, if you or anyone you tell me about is at risk of harm I may have to pass this information on to emergency services, GP or social care. You have a right to access on notes written about yourself. I also have regular supervision where I talk about my work but I always keep your identity anonymous.
How will I store your data?
It depends on the data, it’s mainly all stored on paper and locked in a filing cabinet. I transfer password protected information between insurance company’s with specific codes for each client. Your phone number is stored in my mobile phone with your initials added as a contact.
How long will I store your data for and how will I dispose?
I’ll keep your session notes and unique code for 7 years.
I’ll shred this document with your personal information on 1 month after our work finishes.
I’ll delete your phone number out of my mobile phone 1 month after our finishes.
Is this different for EAP clients?
Your EAP may have given me more information than I would normally collect, for example your work place and registration numbers. Each EAP is different so it depends on your EAP. I usually need this information when I am sending my invoice to your EAP as a way of recognising your case. This information is usually in the format of an email and I will delete this extra data once your EAP has reimbursed me for our work together.

Do you consent to me using your data in this way? Please write yes or no
